At Innovative Integrative Health, we are dedicated to providing comprehensive healthcare services to underserved senior communities. As a Nurse Practitioner, you will play a vital role in enabling our participants to live independently at home and in their communities for as long as possible.
Key Responsibilities:
We are seeking an experienced and compassionate Nurse Practitioner to join our team. Your primary responsibilities will include:
• Performing initial assessments, developing and implementing therapeutic plans of care, and monitoring patient progress.
• Providing primary care to participants in accordance with professional standards and regulations.
• Participating in the Interdisciplinary Team, attending meetings, and consulting with nurses and other staff as medical care issues arise.
• Administering or ordering diagnostic tests and interpreting test results.
• Performing therapeutic procedures, prescribing medications, dispensing supplies, and recommending treatments to manage acute and chronic health problems.
• Documenting and maintaining accurate patient records of services provided.
Nurse Practitioner Requirements:
To be successful in this role, you must have:
• A graduate degree from an accredited Nurse Practitioner program.
• Current licensure as a Registered Nurse and Nurse Practitioner by the California Board of Registered Nursing.
• DEA certification within six months of hire.
• Five years of nursing experience, with at least one year of experience working with frail or elderly populations.
• Supervisory experience is preferred.
• Meet the standards set by the Board of Registered Nursing and the relevant Business and Professional Code of the State of California.
• Completion of CPR (BLS) and first aid training.
• TB screening and a successful health examination by a hospital-designated nurse practitioner or physician are required for employment.
