Behavioral Health Case Manager

Siouxland Community Health Center is seeking a skilled Behavioral Health Case Manager to join our team.
The ideal candidate will have a strong background in case management and a passion for helping individuals overcome mental health and substance abuse challenges.
Key Responsibilities:
- Educate and motivate patients on prevention and self-management of mental health and substance abuse issues.
- Develop and implement individualized case plans to achieve clinical goals.
- Collaborate with healthcare providers to ensure comprehensive patient care.
- Identify and address socio-economic, cultural, and other barriers to medical care.
- Develop relationships with community resources to support patient needs.
Requirements:
- Bachelor's degree in Social Work or related field.
- Current BLS and Mandatory Reporter certifications.
- Minimum of two years experience in case management or related field.
- Strong understanding of community resources and ability to develop relationships with external agencies.
