Senior Nursing Care Coordinator

We are seeking an experienced and compassionate Senior Nursing Care Coordinator to join our team at Interim HealthCare of New Hampshire. As a Patient Care Manager - Home Health RN, you will play a vital role in providing exceptional patient care and support to individuals in their homes.

Responsibilities:- Manage a defined caseload of patients, providing direct patient care and comprehensive nursing services.
- Conduct thorough assessments, develop personalized care plans, and implement evidence-based interventions to promote optimal patient outcomes.
- Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure seamless care coordination.
- Provide education and support to patients and their families on disease management, self-care techniques, and medication adherence.
- Monitor patient progress, adjust care plans as needed, and communicate effectively with stakeholders to ensure timely interventions.
To be successful in this role, you must possess:
- A valid nursing license in the State of New Hampshire.
- Current CPR certification (we offer monthly renewal classes).
- Nursing experience, preferably in home health or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently, prioritize tasks, and manage multiple responsibilities.

The estimated annual salary for this role is approximately $75,000-$90,000, depending on experience and qualifications.
