- Deliver counseling, social work, and emotional support services to patients and their families, focusing on their physical, emotional, and spiritual well-being.
- Collaborate with interdisciplinary teams to identify problems and develop solutions that promote patient-centered care.
- Work closely with patients, families, and caregivers to explore realistic options, make informed decisions, and develop problem-solving strategies.
- Stay up-to-date with best practices in social work and palliative care, incorporating evidence-based interventions into your practice.
- Master's Degree in Social Work, Psychology, or Counseling from an accredited institution.
- California Clinical Social Worker License or California Marriage and Family Therapist License upon hire.
- At least one year of experience in a healthcare setting or related field.
- Training in family counseling and case management.
