Key Responsibilities:- Conduct in-home assessments, including medical history review, medication reconciliation, and comprehensive physical exams
- Perform evidence-based practice screenings, including point-of-care testing
- Identify diagnoses and develop care plans to address member needs
- Communicate findings with primary care physicians and other healthcare providers
- Provide education and support to members on disease management and medication adherence
- Active NP or PA license; ability to obtain by start date
- ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care; ability to obtain by start date
- Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions
- Reliable transportation for home visits
- Fluency in Cantonese
- 1+ years of clinical experience in a similar setting
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home health care or home visit experience
- Ability to travel throughout the state, up to 5% of the time
