Key Responsibilities:
  • Conduct in-home assessments, including medical history review, medication reconciliation, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing

  • Communicate findings with primary care physicians and other healthcare providers
  • Provide education and support to members on disease management and medication adherence
Requirements:
  • Active NP or PA license; ability to obtain by start date
  • ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care; ability to obtain by start date
  • Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions
  • Reliable transportation for home visits
  • Fluency in Cantonese
Preferred Qualifications:
  • 1+ years of clinical experience in a similar setting
  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home health care or home visit experience
  • Ability to travel throughout the state, up to 5% of the time
