Responsibilities:- Conduct thorough nursing assessments of patients and develop individualized care plans to meet their unique needs.
- Provide high-quality patient care, including administering medications, wound care, and other treatments as ordered by physicians.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
- Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
- Document patient progress and outcomes in a timely and accurate manner.
- Current RN license in good standing.
- At least one year of experience as a RN, with a minimum of six months of recent Home Health experience.
- Current BLS certification.
- Valid, unrestricted driver's license and auto insurance.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
