Job Summary:

We are seeking a skilled Hospice Licensed Practical Nurse (LPN) to join our team on a PRN basis. As a key member of our care team, you will provide direct nursing services to patients in accordance with their Plan of Care, under the guidance of our RN Case Manager. Your expertise will empower caregivers, build trusting relationships with patients and their families, and contribute to a culture of compassion and inclusivity.

Responsibilities:
  • Provide skilled nursing services to patients in their homes, in accordance with their Plan of Care.
  • Collaborate with the care team to deliver comfort, peace, and dignity to patients and their families.
  • Empower caregivers by educating them on care for their loved ones.
  • Build trusting relationships with patients and their families, fostering a sense of belonging and community.
Requirements:
  • LVN (Licensed Vocational Nurse) or LPN (Licensed Practical Nurse) licensure.
