Key Responsibilities:
  • Develop and implement personalized care plans for residents based on their unique needs and preferences.
  • Provide direct care to residents, including administering medications, performing treatments, and assisting with daily living activities.
  • Communicate effectively with residents, families, and other healthcare professionals to ensure seamless care transitions and optimal outcomes.
  • Participate in interdisciplinary teams to develop and implement care plans that address the physical, emotional, and social needs of residents.
  • Document resident care and services provided in accordance with regulatory requirements and company policies.


Requirements:
  • A valid Registered Nurse (RN) license and current CPR certification.
  • Experience in long-term care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of laws, regulations, and guidelines related to long-term care.
