At Abode Care Partners, we are seeking a highly skilled House Calls Nurse Practitioner to join our team in Hampton, VA and Newport News, VA. As a critical member of our interdisciplinary care team, you will provide exceptional medical care to seniors and patients with complex needs in their homes or residential facilities.

About the Role

This is an exciting opportunity for a compassionate and dedicated Nurse Practitioner to work in a home health care setting, making a meaningful impact on the quality of life for your patients. You will be responsible for performing evidence-based assessments, collaborating with healthcare teams, and managing patient medical records to ensure holistic care.

Key Responsibilities
  • Comprehensive Medical Assessments: Conduct thorough medical evaluations, including examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses.
  • Collaboration and Communication: Work closely with caregivers and other healthcare professionals to provide interdisciplinary care and maintain a comprehensive plan of care.
  • Patient Record Management: Ensure accurate and up-to-date patient medical records, including ordering and interpreting laboratory and diagnostic testing as needed.
Requirements
  • Licensure and Certification: Active state Nurse Practitioners license, Current Nurse Practitioner National Board Certification, and Active state DEA licensure.
  • Education: Master's degree in Nursing (MSN).
  • Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Skills: Strong interpersonal communication skills – verbal and written, Excellent time management skills with accountability for practitioner's daily workflow, Proficiency in medical instruments and equipment, and Strong knowledge base of HIPAA and OSHA regulations.
