Key Responsibilities:
- Coordinate patient services and liaise with patients, families, and care teams.
- Schedule agency personnel to maintain productivity and ensure efficient care delivery.
- Equip caregivers with necessary information to provide appropriate patient care.
- Deliver direct care to patients in their homes as required.
Qualifications:
- Current Vocational/Practical Nurse License and valid state driver's license.
- Minimum of one year of experience as a Licensed Vocational/Practical Nurse in a clinical setting.
- Preferred experience in home health care.
