Job Title: Pharmacist

We are seeking a highly skilled and experienced Pharmacist to join our team at Westchester Medical Center. As a key member of our pharmacy department, you will play a vital role in providing comprehensive pharmaceutical services to our patients.

Responsibilities:
  • Provide timely and accurate computer order entry of physician orders, prioritizing orders by type and utilizing techniques to minimize potential for order entry errors.
  • Clinically monitor patients' medication regimens for proper drug selection, appropriate doses, interactions, and adverse effects, documenting interventions through Pharmaceutical Care Notes.
  • Provide IV admixtures services in a timely and accurate manner, including preparation of chemotherapy, TPN, routine, and specialty IV products according to department policy and established professional standards.
  • Check completed unit dose cassettes in a timely and accurate manner, adding missing doses, replacing incorrect medications, strengths or forms, and removing excessive or unauthorized medications or doses.
  • Participate in clinical pharmacy programs of the department and the Pharmacy, Therapeutics, and Nutrition Committee, documenting results and outcomes through Pharmaceutical Care Notes.
  • Perform other pharmacist duties as assigned or needed, including filling employee prescriptions, participating in department QA/QI/CQI programs, and maintaining accurate and appropriate record keeping when dispensing controlled substances.
Qualifications/Requirements:
  • Acute-care experience preferred, with comparable hospital pharmacy skills.
  • Bachelor's Degree in Pharmacy from an accredited school of pharmacy or Doctor of Pharmacy.
  • Current NYS license (Pharmacist) and registration with the State Education Department of NY.
  • Good communication skills, both verbal and written.
Preferred Qualifications:
  • IV-Admixture, hyper alimentation, chemotherapy, and computer skills.
