In-Home Care Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our In-Home Care Team at Johns Hopkins Medicine. As a Nurse Case Manager, you will be responsible for delivering, coordinating, teaching, and overseeing nursing care for patients in their homes.
Key Responsibilities- Develop and implement care plans for patients requiring in-home health and support services
- Provide specialized treatments and procedures, educate patients and caregivers, and conduct preventive health screenings
- Coordinate services within the agency and community, and advocate for patients to achieve health-related goals
- Ensure that services are adequately provided, including scheduling visits based on patient needs and care priorities
- BSN preferred
- RNs without a BSN must enroll in a BSN program within 6 months and complete the degree within 4 years of their start date
- Minimum of 2 – 3 years of RN experience preferred
- Previous critical care experience highly desired
- Home care experience preferred
- Competitive salary range: $39.94 - $56.40 per hour
