Seeking Part-time Primary Care Physician for Home Health Services

About the Role

We are looking for a skilled and compassionate Primary Care Physician to provide high-quality medical care to our patients in their homes. As a member of our team, you will work closely with APRNs/PA's to provide primary care and pain management services. Our physicians are dedicated to delivering patient-centered care, focusing on the unique needs of each individual.

Responsibilities
  • Provide primary care services to patients in their homes, including routine check-ups, medication management, and chronic disease management.
  • Collaborate with APRNs/PA's to provide comprehensive care, including pain management and specialist services.
  • Conduct thorough assessments and develop personalized care plans to meet the unique needs of each patient.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Active OH medical license.
  • Active DEA license.
  • Strong desire to work with adult and geriatric populations.
