Job Title: LPN Home Health

Assured Nursing Inc is seeking a skilled LPN to join our team in Lacey, Washington.

Job Summary:

We are looking for a dedicated and compassionate LPN to provide high-quality care to our patients in the comfort of their own homes. As a Home Health LPN, you will be responsible for delivering personalized care, monitoring patient progress, and collaborating with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
  • Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and address any concerns or questions.
  • Document patient progress, including vital signs, medication administration, and treatment plans, in a timely and accurate manner.
Requirements:
  • Current state license as an LPN/LVN.
  • At least 2 years of recent experience as a Home Health LPN.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
  • Basic computer skills, including proficiency in electronic medical records (EMRs).
