Responsibilities
  • Provide skilled nursing care to patients in their homes, including medication administration, wound care, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
Requirements
  • Valid Nursing License in the State of New Hampshire.
  • One year of experience in home healthcare or a related field.
  • Current CPR Certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
