About the RoleWe are seeking a compassionate and dedicated Nurse Care Specialist to join our team at Interim HealthCare - New Hampshire. As a Nurse Care Specialist, you will be responsible for providing high-quality patient care and support to individuals in their homes.
Responsibilities- Provide skilled nursing care to patients in their homes, including medication administration, wound care, and vital sign monitoring.
- Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Valid Nursing License in the State of New Hampshire.
- One year of experience in home healthcare or a related field.
- Current CPR Certification.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- A competitive hourly rate of $29.00-$40.00, depending on experience and licensure.
