Job Summary
We are seeking a highly motivated and experienced PRN RN Home Health to join our team. As a Nurse Telehealth RN, you will play a vital role in providing coordinated skilled nursing care to patients of all age groups in their homes.
Responsibilities
- Assess patients' physical, psychosocial, and environmental needs to develop individualized care plans
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
- Collaborate with the interdisciplinary team to establish and achieve patient goals
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
- Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services
Requirements
- Graduate of an accredited school of professional nursing
- Minimum of one or more years of home health, public nursing, or acute hospital nursing experience
- Familiarity with Medicare home health regulations, documentation requirements, ICD-10 coding, and PPS
- Current BCLS Certification prior to providing patient care
