Job Description

Trinity Health is seeking a skilled and compassionate Per Diem RN to join our home care team. This dynamic position offers a rewarding opportunity to work directly with an interdisciplinary team in the homecare setting.

Key Responsibilities:

Provide high-quality patient care to one patient at a time

Collaborate with the interdisciplinary team to develop and implement individualized care plans

Educate patients and their families on self-management of chronic illnesses

Travel to patient locations to provide care and support

Work closely with other healthcare professionals to ensure seamless transitions in care

Requirements:

1 year of experience in acute care, rehab, or homecare

Valid NYS RN and DMV licensure

Strong communication and critical thinking skills

Pay Range: $36.50 - $49.95

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
