VNS Health is seeking a skilled Physical Therapist to join our team as a Home Care Specialist. As a key member of our interdisciplinary care team, you will provide personalized rehabilitation therapy to patients in their homes or care facilities.
Key Responsibilities- Conduct comprehensive assessments of patients to determine their needs and develop effective treatment plans.
- Deliver high-quality rehabilitation therapy to patients, constantly evaluating their evolving needs and adjusting treatment plans accordingly.
- Practice independently in the community, working collaboratively with other healthcare professionals to ensure seamless care.
- Minimum of one year of physical therapy experience or successful completion of a probationary period as a VNS Health Rehab Intern.
- New York State License and current registration in Physical Therapy.
- Valid driver's license required.
- Competitive pay rate per visit: $70-$75.
