As a part of Almost Family, a leading provider of home healthcare services, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities- Provide high-quality clinical services to patients in their homes, following the plan of care and in coordination with the healthcare team.
- Document patient visits thoroughly and accurately in the electronic medical record, adhering to regulatory and payer requirements.
- Participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with patients, families, physicians, and other healthcare professionals.
- Ensure the confidentiality of patient health information and maintain a professional demeanor at all times.
- Current LPN licensure in the state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification.
