Job SummaryWe are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment Specialist. As a key member of our clinical team, you will conduct comprehensive health assessments, provide medical history reviews, and develop treatment plans to improve patient outcomes.
Responsibilities- Conduct adult/geriatric assessments, medical history reviews, diagnosis, and treatment planning
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to provide comprehensive care
- Develop and implement individualized treatment plans to address patient needs
- Provide education and support to patients and their families to promote healthy behaviors and disease management
- Document patient information and treatment plans in accordance with Matrix Medical Network guidelines
- Master's degree in Nursing or related field
- Current RN and NP licensure in state of practice with prescription authority
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent verbal and written communication skills
