Job SummaryThis is a full-time position located at a Community Health Center in Greenville, OH, where you'll play a critical role in our mission to make world-class specialty care accessible and affordable to all.
Key Responsibilities- Act as the senior clinical staff member at the specialty pharmacy, providing clinical guidance and oversight.
- Lead the pharmacy's Quality Management Committee and oversee the Quality Management Program, implementing and reporting on Quality Improvement Projects.
- Provide exceptional patient care and service to health center providers, actively participating in the Patient Management Program and offering clinical pharmacy services.
- Review and be familiar with clinical guidelines and literature necessary to manage patients' specialty care, becoming a valuable resource for providers and patients.
Additional responsibilities include reviewing patients' therapy for therapeutic duplications, drug interactions, and potential allergies, as well as recommending medication alternatives, consolidation, and dose modification consistent with updated clinical guidelines.
Requirements- PharmD, BS in Pharmacy, or equivalent FPGEC (NABP) degree.
- Unrestricted pharmacist license in good standing in the state of Ohio, with the ability to obtain and maintain licenses in multiple states.
- 3+ years of direct patient care experience in pharmacy, with a strong background in specialty pharmacy experience and a preference for PGY1 residence or equivalent work experience.
As a highly skilled pharmacist, you'll have a strong understanding of clinical pharmacy services, quality management, and patient care, with excellent communication and leadership skills to collaborate with our team and provide exceptional care to our patients.
