Responsibilities:- Patient Assessment and Treatment Planning: The Clinical Social Worker will be responsible for ongoing assessment and formulation of patient needs, ensuring timely completion of treatment plans that meet clinical and regulatory requirements.
- Clinical Services: The position requires providing clinical services to patients and family members as indicated, taking into account the age and specific needs of each individual.
- Liaison and Discharge Planning: As primary liaison, the Clinical Social Worker will work with family and significant others, referrers, and community agencies to develop comprehensive discharge plans, ensuring access to necessary services for patients and their families.
- Collaboration and Utilization Management: The Clinical Social Worker will collaborate with the Utilization Manager to manage patient care in a manner that is both clinically and fiscally sound, working with managed care organizations as necessary.
- Documentation and Paperwork: The position requires ensuring timely completion of all documentation and required paperwork needed for patient evaluation, transition, and billing.
- Education: A Master's degree in Social Work (MSW) from an accredited program.
- Licensure: LICSW licensure in the state of MA (or equivalent independent social work licensure in the state where services are being provided).
- Experience: A minimum of two years post-degree experience in an inpatient, partial, or outpatient setting, with a broad range of clinical skills and experience in managing patient care in a rapidly changing system.
- Estimated Salary: $80,000 - $110,000 per year, based on experience and qualifications.
