- Develop and implement individualized treatment plans to meet patient needs
- Collaborate with healthcare teams to ensure comprehensive care
- Meet productivity requirements of 30 points per 40-hour week
- 1 year of experience in Home Health
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- $2600-$2900 weekly pay
- $0.42/mile mileage reimbursement for patient-related travel
- Monday-Friday schedule
