At Grace Reliant Health Services LLC-1, we are seeking an experienced Physical Therapist to join our team in providing high-quality home health services. This role involves delivering skilled physical therapy services on an intermittent basis to patients in their homes as per physician orders.
Key Responsibilities- Conduct comprehensive assessments of patient needs and develop individualized treatment plans in consultation with physicians.
- Treat patients to relieve pain, restore function, and maintain optimal performance.
- Educate patients and families on the physical therapy program, ensuring effective participation and progress monitoring.
- Maintain accurate records of patient evaluations, treatments, and responses, reporting any changes in condition to physicians.
- Collaborate with staff, patients, and physicians in discharge planning activities, completing a Physical Therapy Discharge summary.
- Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
- Licensed or Registered by the State of Missouri.
- Professional Liability Insurance.
- At least 1 year of professional experience in Physical Therapy.
- A valid Driver's License and Car Insurance.
- A reliable Vehicle.
- This is an outpatient position within Home Health, focusing on Physical & Rehabilitation Medicine.
- The schedule will be Monday to Friday, with flexibility to accommodate patients' needs.
- The ideal candidate must be able to commute or relocate to Cape Girardeau County, MO, before starting work.
We offer a competitive salary of $42.50 - $46.50 per hour, reflecting the expected range based on location and experience. Additional benefits include a flexible schedule, allowing for a better work-life balance.
