About the Role:

We are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager. As a key member of our home care team, you will provide high-quality nursing care to our clients in their homes, while also coordinating their care and services.

Responsibilities:
  • Provide direct care and case management to assigned clients, including physical assessments, medication management, and home safety evaluations.
  • Collaborate with our team to identify and correct issues, and to improve services.
  • Plan, implement, and evaluate care provided to clients, ensuring their needs are met and their care is coordinated.
  • Participate in client care conferences, and serve as a local on-site clinical resource as needed.
  • Assist with marketing activities, such as visiting with clients or physicians to discuss our programs.
Requirements:
  • Graduate of a state-approved school of professional registered nursing.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two years of nursing experience, including one year in home care or a closely related field.
  • One year of supervisory and/or case management experience preferred.
  • Current CPR certification.
