Job Title: Home Health Aide/Personal Care Assistant

Lutheran Services is seeking a compassionate and caring Certified Home Health Aide (HHA) or Personal Care Assistant (PCA) for a full-time position to meet the needs of our 3rd shift (10pm - 6am).

Job Summary:

Our HHAs and PCAs provide direct care to residents as indicated by the individual care plan, under the supervision of the RN Case Manager in accordance with facility policies and procedures and report resident needs and concerns to the RN Case Manager.

Responsibilities:
  • Provide direct care to residents as indicated by the individual care plan
  • Report resident needs and concerns to the RN Case Manager
  • Complete medication administration training
  • Assist with personal hygiene according to the resident's care plan
  • Identify resident problems and concerns and report them immediately to the Charge Person
  • Conduct resident rounds as assigned
  • Assure residents have call lights at hand and answer call lights promptly
  • Implement corrective action as necessary under the direction of RN Case Manager
  • Report and record incidents and/or unusual problems according to established facility procedures
  • Report changes in residents' conditions to RN Case Manager
  • Complete documentation utilizing computerized medical record system accurately, timely and following facility policies and procedures
  • Refer resident and family complaints and problems to Charge person
  • Assure residents are monitored with meals, nourishment and transportation to meal service area
  • Provide housekeeping services to residents
  • Provide other supportive tasks for residents as needed
  • Follow facility procedures for admission, discharge and transfer of residents
Requirements:
  • High school diploma or equivalent required
  • Must have HHA or PCA Certification, which means the successful completion of New York State training for home health aide services or equivalent exam approved by New York State is required
  • Knowledge of data entry in computer for medical records

Salary Range: $17.55 Min to $24.79 Max
