About the Role

We are seeking a skilled Nurse Care Specialist to join our team at Rochester Regional Health. As a key member of our Home Care department, you will play a vital role in providing high-quality patient care and support to individuals in their own homes.

Key Responsibilities
  • Patient Care and Service: Provide comprehensive assessments of patients' bio-psycho-social needs, perform skilled nursing care, and prescribed treatments as ordered by physicians.
  • Planning: Develop and document individualized care plans tailored to each patient's unique needs, in collaboration with the interdisciplinary health team.
  • Communication: Maintain effective communication with patients, families, office staff, and supervisory personnel to convey patient health status, treatment plans, and progress.
  • Documentation: Accurately and completely document patient care, including physician orders and program standards.
  • Compliance: Adhere to department and system protocols, regulations, and education requirements.
Requirements
  • Education: Associate's Degree in Nursing (Required); Bachelor's Degree in Nursing preferred.
  • Experience: 1 year of nursing experience preferred, with prior home health, clinical, and direct patient care experience.
  • Attributes: Ability to work independently, with strong communication and interpersonal skills.
