**Job Title:** Care Manager

**Location:** West Central Nebraska Area Agency on Aging

We are seeking a dedicated and compassionate Care Manager to join our team. In this role, you will be responsible for assessing the needs of elderly clients and their families, coordinating community-based services, and implementing care plans.

Salary: $21-$22 per hour

About the Role:

The successful candidate will possess excellent communication skills, a strong understanding of long-term care and gerontology, and experience in community health. You will work closely with our team to provide high-quality care to our clients, while also meeting the requirements of the Nebraska Care Management program.

Key Responsibilities:

Assess the needs of elderly clients and their familiesCoordinate community-based servicesImplement care plansWork with Resource Development to secure service providersAuthorize providers and services as neededMonitor and evaluate service deliveryAdvocate for clientsMaintain confidentiality of client informationComply with all reporting requirementsAttend trainings and meetings as required

Qualifications:

  • Current Nebraska license as a Registered Nurse, or baccalaureate or graduate degree in the human services field, or certification under the Nebraska Social Work Law
  • At least two years of experience in long-term care, gerontology, or community health
  • Proficient in written and oral communication skills
  • Basic computer skills
  • Valid driver's license
  • Proof of insurability for malpractice liability insurance
