We are seeking a skilled LPN to join our team as a Home Health Clinical Liaison. This role is responsible for creating a seamless transition for patients from hospital to home health care, ensuring their needs are met and they receive the best possible care.
Key Responsibilities- Develop and maintain positive relationships with hospital teams, healthcare providers, and other key stakeholders.
- Facilitate effective communication between patients, families, and caregivers to ensure a safe and successful transition to home health care.
- Provide individualized service delivery, addressing patient and caregiver needs throughout the transition process.
- Manage referrals from hospital Case Management to home health agencies, ensuring patients meet CMS criteria for home health services.
- Ensure required documentation is sent with referrals to support billable episodes of care.
- Serve as a resource to hospital teams regarding home health services and provide educational in-services as requested.
- Minimum two years of experience in home health required.
- Experience working in transitional care or case management preferred.
- Licensed Practical/Vocational Nurse (LPN/LVN), or Registered Nurse, or Licensed Physical Therapy Assistant (LPTA) required.
