We are seeking a dedicated and compassionate Licensed Social Worker to join our team at Optalis Healthcare in Canton. As a key member of our interdisciplinary team, you will play a vital role in providing support and assistance to residents and their families in our long-term care setting.

Responsibilities:
  • Conduct thorough assessments of residents' social and emotional needs, developing personalized care plans to address their unique requirements.
  • Collaborate with the interdisciplinary team to ensure seamless care delivery, including counseling and support services.
  • Advocate for residents and their families, promoting their rights and interests.
  • Coordinate discharge planning and community resources to ensure a smooth transition for residents.
  • Participate in regular team meetings and resident care conferences to ensure effective communication and care coordination.
  • Maintain accurate and timely documentation of resident interactions and care plans.
  • Contribute to quality improvement initiatives, driving excellence in care delivery and resident satisfaction.
Requirements:
  • Bachelor's or Master's degree in Social Work or a related field, such as Sociology, Psychology, or Rehabilitation Counseling.
  • Current licensure as a Social Worker in the state of Michigan is preferred.
  • Experience working in a long-term care setting is highly valued.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Proficiency in Microsoft Office and electronic medical records is essential.
