About the Role:

We are seeking a skilled Independent Rehabilitation Specialist to join our team at CommonSpirit Health at Home. As a key member of our interdisciplinary team, you will be responsible for developing and implementing individualized rehabilitation plans that empower patients to regain independence and functionality in their daily lives.

Key Responsibilities:
  1. Conduct thorough assessments of patients' physical abilities and develop targeted rehabilitation plans to address specific deficits and limitations.
  2. Provide one-on-one physical therapy sessions in patients' homes, using evidence-based techniques and interventions to promote optimal outcomes.
  3. Collaborate with other healthcare professionals, family members, and caregivers to ensure coordinated care and successful transition to community-based settings.
Requirements and Qualifications:
  • Bachelor's degree from an accredited institution approved by the American Physical Therapy Association.
  • Active licensure to practice physical therapy in the state served.
  • Minimum one year of experience in teaching or clinical settings.
Compensation and Benefits:
  • A highly competitive hourly rate of $42.75 - $60.19, commensurate with experience and qualifications.
