Job Description

Job Summary:

Pace Consulting LLC is seeking a highly skilled and compassionate Family Case Manager - Behavioral Health Specialist to join our team. As a key member of our School and Community Youth Services department, you will provide culturally sensitive and comprehensive case management services to families and youth in need.

Key Responsibilities:

  • Provide case management services to promote self-sufficiency skills, family development, and linkage to mental health resources for un- and under-insured youth and their families.
  • Facilitate groups to support the services listed above, including support groups and educational workshops.
  • Collaborate with therapists, school staff, and community partners to provide comprehensive care while maintaining strict confidentiality of client information.
  • Organize, coordinate, and facilitate meetings, workshops, presentations, activities, and events to develop partnerships between families and schools.
  • Work closely with service providers to develop a referral system and promote a continuum of care.

Requirements:

  • Bachelor's degree in Social Work (B.S.W.), Psychology, or related field.
  • Experience working within collaborative school-based models of service delivery is preferred.
  • Prior experience/equivalent knowledge of immigrant populations & acculturation issues preferred.
  • Knowledge of community resources in the Montgomery County area preferred.
  • Valid state driver's license with 3 or fewer points at time of hire.

Working Conditions:

  • This position is partially community-based with time allocated to an office setting.
  • Ability to travel to zip code 20837.
  • Ability to maintain moderate physical activity within the community to include walking (including stairs), lifting, and driving.

Reporting Relationships:

Reports to the Program Director.
