Responsibilities Overview

  • Assess and monitor the medical and health requirements of individuals.
  • Conduct weekly visits to assigned residences to ensure nursing oversight and comprehensive evaluations.
  • Collaborate with primary care providers, medical specialists, and various clinical personnel as needed.
  • Serve as an advocate for the healthcare needs of individuals.
  • Maintain consistent communication with the medical liaison or designated personnel.
  • Support residential staff in upholding health services for individuals.
  • Utilize appropriate reporting protocols and chain of command to address individual and/or staff concerns.
  • Complete follow-up documentation for individuals after emergency room or hospital visits.
  • Review and provide feedback on lab results, x-rays, and consultations, ensuring follow-up with practitioners as necessary.
  • Ensure timely documentation of home visit reports.
  • Adhere to pressure wound policies and documentation standards.

Deliver appropriate nursing services and support to Residential/Day Hab staff
  • Respond promptly to emergency requests (both within and outside of assigned caseload).
  • Conduct thorough nursing assessments and related documentation, such as RN Notes and nursing logs, within specified timeframes.
  • Implement individualized nursing interventions.
  • Address staff inquiries regarding individual-specific medical and nursing matters.
  • Ensure that Plans of Nursing Services (PONS) are current and signed by staff, maintaining all records in digital files.
  • Provide health-related training and consultation to day program and residential staff.
  • Annually review the medical section of Individualized Service Plans (ISP) and conduct medication audits for each home.
  • Stay informed about updates to the medical section of ISPs.
  • Responsible for obtaining informed consent from individuals for medications, in accordance with established policies.

Ensure adequate nursing assessment and supervision of Direct Support Assistants (DSAs) regarding individual-specific nursing delegated tasks, including medication administration.
  • Complete initial medication certifications and perform annual clinical evaluations for DSAs on procedures, including medication administration.
  • Provide necessary training for staff on new or revised medication policies.
  • Supply certification and recertification information for the database in a timely manner.
  • Document medication errors and oversee staff remediation for such errors.

Provide clinical supervision to Licensed Practical Nurses (LPNs)
  • Offer adequate nursing supervision and conduct performance evaluations (as assigned) related to individual-specific nursing procedures.

Maintain professional competencies
  • Participate in Nursing Discipline meetings.
  • Complete mandated training and pursue available discipline-specific training online.
  • Identify areas for improvement within personal and nursing department practices and contribute to action plans.
  • Engage in special projects and assignments.
