Responsibilities
As a Registered Nurse Case Manager, your key responsibilities will include:
- Coordinating patient care and ensuring seamless transitions between healthcare providers
- Performing accurate OASIS collection and maintaining up-to-date medication profiles
- Consulting with physicians and office staff to ensure timely and effective patient care
- Attending weekly case conferences and monthly case manager meetings to stay informed and share best practices
Qualifications
To be successful in this role, you'll need:
- A current RN license in the state of employment
- A minimum of one year of clinical experience, preferably in home health care
- Demonstrated knowledge and skill in current nursing practice
- A valid state driver's license and automobile liability insurance
