Westfield Gardens Nursing and Rehab is seeking a dedicated Rehabilitation Specialist to deliver therapeutic services across our various care facilities. The Rehabilitation Specialist will execute rehabilitation programs and provide treatment to enhance patient independence and functionality, under the guidance of a licensed Occupational Therapist.

Available Shift: PRN - Weekdays / Weekends



Key Responsibilities:

- Implement individualized treatment plans as directed by the supervising Occupational Therapist.
- Collaborate with the rehabilitation team to create programs tailored to the unique needs and abilities of each patient.
- Educate patients, families, and caregivers on the techniques and skills involved in the occupational therapy treatment program under supervision.
- Assess residents to gather essential data for treatment planning and develop comprehensive treatment plans that outline target objectives, as well as short-term and long-term goals.


Qualifications:

- Associate's Degree in Occupational Therapy from an accredited institution or possess a valid Occupational Therapist license; OTR/L certification.
