Job SummaryWe are seeking a skilled Family Medicine Physician to join our team. As a key member of our healthcare team, you will provide medical care in accordance with the best current standards of professional practice.
Key Responsibilities- Obtain histories, perform physical examinations, prescribe treatment and medication for conditions falling in the scope of your specialty following evidence-based medicine.
- Provide relevant information about our Patient Centered Medical Home (PCMH) and Chronic Care Management program (CCM) to patients and staff.
- Develop and implement plans of care utilizing sound clinical judgments based on assessment of the physical, psychological, and emotional needs of clients.
- Consult, refer, and collaborate with other disciplines involved in the delivery of total patient care.
- Document all activity utilizing appropriate medical records.
- Participate in ongoing educational programs to maintain clinical skills and knowledge.
- Ensure prompt follow-up action on patients with abnormal test results or physical findings.
- Set standards for preventive care and diagnostic screening within your specialty.
- Medical Doctoral Degree (MD/DO)
- Licensed by and currently registered with the New York State Department of Education
- Basic Life Support certification
