Job Overview:

The Care Coordinator at MountainView Hospital plays a vital role in supporting the organization's initiatives towards clinical integration and accountable care strategies. This position aims to achieve the triple aim in healthcare by improving quality and patient satisfaction, outcomes, and reducing costs.

Key Responsibilities:

  • Identify High-Risk Patients: The Care Coordinator will identify patients with needs for early intervention related to discharge education, transitional care, or disease management and coordinate interventions with primary care physicians (PCPs) and other care providers.
  • Referral Coordination: Refer patients to medical and behavioral health providers as necessary for further intervention.
  • Patient Care Supervision: Work under direct supervision of a nurse supervisor to provide excellent patient care to all clients.
  • ACO/PCMH Transition: Participate in the transition of the clinic into an Accountable Care Organization/Primary Care Medical Home model.
  • Population Health Engagement: Engage with the population health team on Quality-Based programs in the clinics.

Requirements and Qualifications:

  • Certifications and Licenses: Hold a Certificate in Medical Assisting (CMA), Practical Nursing License (LPN), or Graduate with a degree in Registered Nursing (RN).
  • Licensure: Maintain current licensure in Idaho.
  • Certification: Obtain certification in Care/Case Management or Population Health Management within three years after hire.

Preferred Experience:

  • Inpatient and Outpatient Settings: Gain experience in both inpatient and outpatient care settings.
  • Health Plan Experience: Possess health plan experience.
  • Care Management Background: Have past experience in Care/Case Management, Population Health Management, Utilization Review/Management.

Compensation: $65,000 - $85,000 per year
