Estimated salary: $65,000 - $85,000 per year (dependent on location and experience)

Job Description

This position requires a Registered Nurse with a minimum of six months' experience in an acute care setting. As a Nurse Care Coordinator, you will:

  • Maintain accurate and thorough documentation of patient assessments, care plans, and interventions
  • Develop and implement individualized care plans in coordination with patients, families, and interdisciplinary teams
  • Communicate effectively with physicians, nurses, and other healthcare professionals to ensure continuity of care
  • Participate in discharge planning and coordinate post-acute care arrangements as needed

In addition to these responsibilities, you will have the opportunity to develop your leadership skills by participating in the orientation and training of new staff members. You will also contribute to a positive work environment by maintaining a good working relationship with colleagues and adhering to hospital policies and procedures.

Requirements

To be successful in this role, you must possess:

  • A Bachelor's degree in Nursing or Associate degree in Nursing from an accredited program
  • Current state licensure as a Registered Nurse
  • BCLS certification required; ACLS certification preferred
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare teams
