Role Summary:

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team. As a Hospice Nurse, you will play a vital role in providing high-quality care and support to our patients and their families.

Key Responsibilities:

  • Provide direct patient care, including admissions, routine visits, recertifications, discharges, and deaths.
  • Assist families in creating individualized plans of care and participate in care conferences and bereavement plans.
  • Supervise hospice aides and communicate effectively with patients, families, physicians, and interdisciplinary groups.
  • Provide clinical education and ongoing support to patients and families.

Requirements:

  • Active RN license in the state of Minnesota.
  • Current Basic Cardiac Life Support (BCLS) certification.
  • CPR certification or willingness to obtain within 6 months of hire.
  • Valid driver's license and use of an insured personal vehicle.
