Job SummaryThe LPN Telemetry position is responsible for providing nursing care to patients in a telemetry unit. This includes assessing patient needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities- Provide direct patient care, including vital sign monitoring, medication administration, and wound care.
- Assess patient needs and develop individualized care plans to meet those needs.
- Collaborate with other healthcare professionals, including RNs, physicians, and therapists, to ensure optimal patient outcomes.
- Participate in patient education and discharge planning to ensure a smooth transition to home or other healthcare settings.
- Contribute to a positive and supportive work environment by maintaining open communication with colleagues and supervisors.
To be successful in this role, you will need:
- A current LPN license in the state of hire.
- Graduation from an accredited practical nursing program.
- Basic Life Support (BLS) certification.
- One or more of the following certifications: ENPC, TNCC, CPHON, OCN, or Six Assessment & Care Modules.
