Position Overview:

Under the clinical supervision of the Director of Nursing and the administrative oversight of the Director of Residential Services, the Community Nurse Specialist/Nursing Case Manager is responsible for ensuring that health and nursing quality standards are upheld for the residents in our group homes. This role includes providing guidance to direct support staff, delivering direct nursing care, and coordinating external medical services. The Community Nurse will also oversee the operations of the assigned group home facility, as directed by the Residence Manager. Additionally, this position involves evaluating the capabilities of Approved Medication Administration Personnel (AMAP) to ensure compliance with New York State (NYS) regulations regarding medication administration.

Qualifications:

Education:

Must be a graduate of an accredited nursing program.

Experience:

  • Minimum of three to five years of nursing experience.
  • Preferred one to two years of experience working with individuals with developmental disabilities.

Additional Requirements:

  • Current NYS Registered Nurse license.
  • Valid NYS driver's license with the ability to travel between agency locations.
  • Availability to respond to healthcare emergencies 24/7 via pager.

Compensation: $50.00-$56.31, based on experience.
