VNS Health is seeking a skilled Registered Nurse (RN) to lead our Home Health team as a Field Supervisor. As a key member of our clinical team, you will provide expert guidance and support to our Home Health Aides, ensuring exceptional patient care and outcomes.

Key Responsibilities:
  • Conduct regular field visits to patients' homes to assess their needs and provide clinical guidance
  • Supervise and mentor Home Health Aides to ensure they are providing high-quality care
  • Collaborate with the clinical care team to develop and implement effective care plans
  • Document patient visits and care plans in our electronic medical record (EMR)
Requirements:
  • Current license to practice as a Registered Nurse in New York State
  • Minimum of two years of experience as an RN in a home care setting
  • Valid driver's license may be required
