ROLE OVERVIEW
The Physical Therapist (PT) plays a crucial role in assessing the needs of patients referred for home care services. You will develop tailored treatment plans and deliver therapy services as outlined in these plans, collaborating closely with physicians and case managers. This position allows for creativity and flexibility in your approach to patient care.

QUALIFICATIONS REQUIRED
- Graduation from an accredited Baccalaureate, Master's, or Doctorate program in Physical Therapy.
- Current, unrestricted license to practice as a Physical Therapist in the state of employment.
- A minimum of two years of relevant experience as a Physical Therapist, preferably in community or home health settings.
- Strong verbal and written communication skills, along with effective organizational abilities.
- Current CPR certification obtained through an in-person course with practical skills evaluation.
- Access to reliable transportation for home visits.
