Key Responsibilities- Provide direct nursing care to patients in their homes, under the supervision of a registered nurse or physician.
- Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.
- Administer medications, perform wound care, and provide other treatments as prescribed by physicians.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare settings.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal patient outcomes.
- Graduation from an accredited Practical Nurse program.
- Current State of North Carolina LPN licensure or Multi-State Compact License.
- Active and current driver's license.
- BLS Certification.
