Position Overview

As a Registered Nurse (RN) specializing in Care Management, you will play a crucial role in enhancing the quality of life for our patients. Your responsibilities will include delivering comprehensive care to individuals with diverse and complex health conditions, fostering strong relationships with patients and their families.

Key Responsibilities
  • Conduct thorough nursing assessments and collaborate with the healthcare team, patients, and families to formulate effective care plans.
  • Set realistic and measurable short- and long-term goals for patient care, implementing interventions as necessary to reduce hospital readmissions and continuously evaluate outcomes.
  • Delegate nursing tasks to team members and oversee their execution to ensure high standards of care.
  • Facilitate communication of patient information among staff and across shifts to maintain continuity of care.
Qualifications

To be considered for this role, candidates must:

  • Be a graduate of an accredited nursing program and possess a current nursing license from the State Board of Nursing.
