Outpatient Physical Therapist

Position Overview
An exciting opportunity awaits as a member of the FAIRFAX FAMILY PRACTICE team in the role of Outpatient Physical Therapist. In this position, you will engage with dedicated patients eager to regain their functional abilities. Collaborate closely with medical professionals to develop effective treatment strategies and assess patient outcomes.

Work Schedule: Monday to Friday, 8-hour shifts
Key Responsibilities:
- Accurately documents evaluation outcomes in accordance with departmental standards.
- Completes daily progress notes that reflect patient responses, performance, treatment status, and discharge requirements.
- Concise documentation of discharge summaries, treatment progress, follow-up needs, and educational information provided to patients and their families.
- Ensures timely and clear documentation to support reimbursement and continuity of care.
- Records evidence of patient and family education within medical records.
- Communicates effectively with members of the healthcare team and adapts to feedback and changes.
- Provides guidance to support staff in compliance with clinic policies.
- May assume leadership roles within the department or organization as needed.
Qualifications:
Certification - Basic Life Support required upon commencement
Licensure - Virginia Physical Therapist required upon commencement
Experience - Preferred but not mandatory
Education - Bachelor's Degree in Physical Therapy; Master's Degree preferred
