Job Summary:

We are seeking a qualified Licensed Practical Nurse (LPN) to join our Assertive Community Treatment (ACT) team at our Georgetown, Delaware location.

Key Responsibilities:
  • Coordinate behavioral and physical health care needs of program participants
  • Monitor participants' medication and provide psychiatric nursing services
  • Assist in developing, implementing, and evaluating recovery plans
  • Act as a liaison with community agencies and families to maintain coordination of care
  • Provide ongoing assessment of participants' mental health symptoms and respond to recovery plans
  • Educate and supervise participants in self-administration of medication
  • Ensure annual review of participants' physical condition and assist access to and encourage compliance with treatment for identified physical problems
Requirements:
  • Board Certified Licensed LPN in the state of Delaware or participating in the Nursing Licensure Compact
  • Knowledge of mental health systems and supports
  • Knowledge of multiple support systems for substance abusers
  • Good communication skills, both oral and written
  • Ability to work independently and as part of a focused team
  • Possession of a valid driver's license with an acceptable driving record
  • Very strong organizational and analytical skills
Working Conditions:

Frequent talking and listening. Requires the ability to lift, carry, hold, kneel, stack, stoop, reach, and bend. Local travel required, including transporting participants. May be required to report to work during emergencies, including inclement weather. May require crisis intervention in emergency situations. Scheduling flexibility and possible on-call work are necessary.
