Job Summary

Hospice RN Case Managers are responsible for overseeing the care of terminally ill patients. They plan, organize, and direct hospice care utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. Effective interaction with patients, caregivers, families, and other interdisciplinary team members is essential, maintaining standards of professional nursing and clinical competency.

The RN Case Manager will provide direct and indirect skilled comprehensive nursing care to meet the needs of patients and families within their homes and communities. This includes providing dietary counseling, initiating nursing procedures, and supervising and training other nursing service personnel.

Responsibilities
  • Completes an initial, comprehensive, and ongoing assessment of patient and family in conjunction with the IDG to determine and meet the needs of the hospice patient and family.
  • Provides a complete physical assessment and history of current and previous illness and initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Develops a plan of care that establishes goals, based on nursing diagnosis and incorporates hospice and palliative nursing actions.
  • Coordinates the implementation of the plan of care for patients residing in ALF, SNF, NF, or ICF.
  • Includes the patient and the family in the planning process.
  • Provides hospice nursing services, treatments, and preventive procedures.
  • Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient's physical or emotional condition.
  • Teaches, supervises, and counsels the hospice patient and family members about providing care for the patient.
  • Supervises and trains other nursing service personnel.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor's orders.
  • Attends IDG conferences.
  • Maintains records as required by Hospice.
  • Follows the policies and procedures of Hospice. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
  • Always communicates to the supervisor if unable to meet a patient's need or perform a procedure.
  • Participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
  • Maintain skills and knowledge as required by state regulatory practices.
  • Complies with agency infection control policies and procedures.
Requirements
  • Graduate from an accredited school of nursing and has a current/active license as a registered nurse to practice in the state.
  • Ability to work independently, make accurate, and at times, quick decisions.
  • Ability to respond appropriately to crisis outside of a hospital setting.
  • Acceptance of and adaptability to different social, racial, cultural, and religious modes.
  • Completes Hospice training program.
  • Minimum 2 years of experience as a registered nurse, preferred.
Working Conditions

Be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids, and excrements, adverse environmental conditions, and hazardous materials.

Must have a current valid driver's license, auto liability insurance, and reliable transportation.
