Delhi Post Acute is seeking a dedicated Speech Therapist to join our team on a part-time basis. As a Speech Therapist, you will be responsible for evaluating and treating patients, effectively communicating with families, physicians, and health team members, and maintaining accurate documentation of services in medical records.
Key Responsibilities:- Evaluate patients within 24 hours of physician referral and document the cause for delay in the medical record if unable to complete the evaluation within this timeframe.
- Develop effective treatment plans and obtain approval for services from referring physicians.
- Implement treatment plans as directed by physicians.
- Assist the nursing department with training of Restorative Aides.
- Communicate regularly with supervisors and health team members regarding patient progress, issues, and plans.
- Engage in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
- Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation per state regulations and payer requirements.
- Educate patients' families or nursing staff on maintenance programs and caregiver training for discharge preparation.
- Participate in discharge planning to ensure a smooth transition for patients.
- Report any issues with department equipment to maintain it in good working order.
- Relate positively, effectively, and appropriately with patients, families, staff, and colleagues.
- Licensed as a Speech Therapist in the state, with ongoing continuing education requirements met.
- Ability to read technical procedures, comprehend policy and procedure manuals, and effectively present information.
- Proficient in applying concepts such as fractions, percentages, ratios, and proportions in practical situations.
- Capable of solving practical problems and interpreting a variety of written, oral, or diagram instructions.
