Nurse Practitioner

As a Home Therapies Registered Nurse at FRESENIUS MEDICAL CENTER, you will play a vital role in managing patient care in an independent home therapy program. Your primary responsibility will be to provide education and support to patients and their caregivers who have chosen to dialyze in the comfort of their own homes.
Key Responsibilities- Participate in decision-making, teaching, leadership, and quality improvement activities that enhance patient care outcomes and home therapies program operations.
- Identify and evaluate candidates for the Home Therapies Program, including assessing patients' home environment if necessary.
- Perform assessments and identify barriers to the Home Dialysis candidate's home environment and caregiver readiness and ability to perform dialysis treatments in the home.
- Admit new patients to the Home Therapies Program using the appropriate processes.
- Coordinate patient care, including assessment, planning, intervention, and evaluation for an assigned group of hemodialysis patients.
- Assess and manage patients' response to home dialysis training and treatment therapy by following prescribed predetermined protocols.
- Analyze dialysis data for Home Therapy patients and document and adjust or modify the treatment plan as needed.
- Provide initial and ongoing education to patients and their families on home dialysis treatment and the operation and maintenance of all home therapies equipment.
- Ensure patient awareness related to transplant and treatment modality options.
- Administer medications as prescribed or in accordance with approved algorithm(s), and document appropriate medical justification and effectiveness.
- Initiate or assist with emergency response measures.
- Complete routine patient evaluations within the clinic setting.
- Ensure correct laboratory collection, processing, and shipping procedures are performed.
- Collaborate with the Interdisciplinary Team on the patient care plan.
- Make referrals to Social Worker and Registered Dietitian as appropriate.
- Complete CAP requirements to either maintain or advance within the program.
- Graduate of an accredited School of Nursing.
- Current appropriate state licensure.
- Current or successful completion of CPR BLS Certification.
- Must meet the practice requirements in the state in which he or she is employed.
- Qualities and traits: compassion, caring, support for colleagues, collaborative, reliable.
- Experience as a Registered Nurse (preferred but not required).
- Chronic/home hemodialysis experience (preferred but not required).
- Successfully pass the Ishihara Color Blind Test.
